It is certainly not an issue with your modem.
All is going very well up to this point:

"Provider sent a request to terminate the connection ..."

That basically means that your provider hung up on you.
I see this when I'm using an old SIM card which has expired, or I enter a wrong APN.

It can also be a limitation of your plan that does not allow you to do certain things, and the other side has detected something.

Can you give some information about your provider/plan?

Edit: I see that the hang-up happens during the authorization phase. I'll check later (got to go to work now).

I can see that you entered "yemenmobile" as user and password.

Are you sure this is correct?
I have found this page when googling, and it says to use "ymobile" as username and as password:

It connected successfully but no apps can connect to internet. Also it uese the sim card inside mobile. Not usb. When put mobile in plane mode make restart. Removed the sim and still no apps got internet access.

If the internal phone modem is used by PPP Widget, it must be added to the "ignore list".

Can you post a USB log where you check for a modem but with the EC176 not connected?

There we have it!

Now go to the configuration of PPP Widget and look for the option "List of USB devices to ignore" - it's under "Troubleshooting".
Select the option and type "05c6:904c", then press "OK".

There may be a difference when you next connect your modem.
